There are three ways to obtain a home collection kit from AGI.

1) Fill out the secure online order form.

2) Call us at 1-800-362-5559 and request a kit. A credit card will be necessary to purchase your kit in this manner. You will be instructed how to complete your transaction. We will send you the home collection kit with complete instructions as soon as payment has been approved.

3) Fill out the mail order form and mail or fax (801-582-8460) it to our office. You may send your payment with the order form by: money order, MasterCard or Visa, or a personal check (allow 7 days for checks to clear)
